behavioral health model. LCSW, LCMHC or Licensed Masters

DSM-IV-TR and knowledge of primary care behavioral health needs
preferred.

CALEDONIA HOME HEALTH CARE
Physical Therapist
Provide Physical Therapy services to homebound clients
throughout our service area. Must be able to work with a team,
as well as independently.
Applicants must have a Vermont Driver’s license, reliable
transportation and Vermont licensure as a Physical Therapist
with at least one year of experience in a health care setting.
Every other Weekend Community Health Staff Nurse, RN
PT position with weekend rotation providing home care services
in Caledonia and southern Essex counties. Requirements include
having a valid driver’s license, a reliable vehicle, and
licensure in the State of Vermont as an RN.

Full
time Home Health and Hospice Administrator
The qualified candidate will serve as the home health and hospice
clinical director, and will provide the overall leadership to the
clinical operations. An RN or Master's degree in a health care
related field with 5 years of hospice or home health care experience
is required. Must have knowledge of state and federal regulations,
compliance, QA/QI, coordination and delivery of care, and is
skillful in the management and analysis of complex financial and
operational data.
