The Concord Health Center is a family practice designed to make health care and health education more readily available to area residents.

We offer complete, coordinated, and individualized health care to people of all ages; from babies to great-grandparents. In this sense, we try to fill the role of the old-time family doctor.

We have designed our Health Center to include the most modern approaches to health care. We provide a one-stop location for many of your family's health needs, including primary medical care, behavioral health, laboratory tests, and a variety of programs geared toward health maintenance - the prevention of illness.

Our staff will work with you to treat you when you are sick, and to keep you healthy when you are feeling well.

We offer diagnosis and treatment of illness in children, adolescents and adults, as well as preventive health care. We assist patients in managing their chronic illnesses, and make referrals to specialists as needed.

    Insurance and Payment For Services
    Co-payments for services represent an agreement between you and your insurance provider.
    All co-pays are due at time of services. Please bring your insurance card with you to your visit, so that we can verify coverage.
    Payment options
    NCHC will directly bill many insurance companies. In addition, through our sliding scale program we offer reduced fees for those who meet income guidelines. Cash, check and major credit cards are all accepted for payment.
    No Show Policy
    Patients who do not show up for appointments and do not call in advance to cancel or reschedule may lose their right to make advance appointments.

    Sarah Berrian, MD, received her Bachelor of Arts in Biochemistry from the Mount Holyoke College. She attended the University of Vermont for her MD, interned at Carney Hospital in Boston Massachusetts, and completed a medical residency program at the University of Rochester. She was board certified in family practice in 2000. She has been providing health care to the Concord area since 2000.
    Susan Taney, NP, is a certified Family Nurse Practitioner. She received her Bachelor of Science degree from the College of Misericordia in Dallas, PA. She received her master of Science in Primary Care from the University of Pennsylvania in 1990. She was certified as an Adult Nurse Practitioner in October 1990. She has been serving the Concord area community since the Fall of 1993.
    Mary Ready, MD, received her Bachelor of Arts degree from Yale University. she attended the University of Vermont for her Medical Degree and completed her Family Medicine residence at the Maine Dartmouth program in Waterville, Maine. She was board certified in Family Practice in 2002. She has been providing health care to the Concord area since 2005.
    Donna Ransmeier, Care Coordinator - Behavioral Health Consultant. Received a B.A. in Environmental Studies from Lyndon State College; a B.S. in Human Services from Springfield College and her M.S. in Clinical Psychology from Springfield College. Ms. Ransmeier joined the Concord Health Center in August 2003.
